We will not sell your health information. We will obtain written authorization before using or sharing it for purposes not otherwise permitted by law. You may revoke an authorization in writing at any time, except where action has already been taken.

Family Members and Caregivers

With your permission, we may share relevant information with family members, representatives, or others involved in your care. If you cannot communicate your preference, we may share limited information when legally permitted and reasonably considered to be in your best interest.

Filing a Complaint

You may contact Daybreak Vantage Care if you believe your privacy rights have been violated. To file a complaint you can reach out to Connecticut Department of Consumer Protection at https://portal.ct.gov/dcp/file-a-complaint

You will not be denied services or treated differently for filing a complaint.
