5. Care Consultations

A consultation does not:

  • Guarantee acceptance as a client;
  • Guarantee caregiver availability;
  • Establish a service agreement;
  • Replace a professional medical evaluation;
  • Guarantee a specific care outcome;
  • Create a binding obligation to provide services.

Recommendations are based on the information available at the time of the consultation. Families are responsible for providing accurate and complete information regarding the prospective client’s condition, needs, routines, risks, mobility, safety concerns, and scheduling requirements.

Daybreak Vantage Care may recommend that a family consult a physician, nurse, therapist, emergency service, or another qualified professional when needs fall outside the scope of non-medical home care.

6. Consultation and Contact Forms

When submitting a contact or consultation form, you agree to provide information that is accurate, current, and complete to the best of your knowledge.

You must not knowingly submit false, misleading, unlawful, defamatory, fraudulent, or harmful information.

Submitting a form does not guarantee:

  • A response within a specific time;
  • Acceptance of a service request;
  • Immediate caregiver availability;
  • Confirmation of an appointment;
  • Formation of a care agreement.

Daybreak Vantage Care aims to respond to consultation requests promptly, but response times may vary due to business hours, weekends, holidays, emergencies, staffing, or technical issues.

Please do not submit Social Security numbers, banking credentials, credit card details, complete medical records, or other highly sensitive information through a general website form unless Daybreak Vantage Care has specifically instructed you to use an approved secure method.

Daybreak Vantage Care serves New Milford, Danbury, Brookfield, Waterbury, Southbury, Roxbury, and surrounding Connecticut communities.

The appearance of a city or town on the website does not guarantee immediate or continuous service availability in that area.

Coverage may depend on:
